I understand and agree that, by logging into and using ChemTutor, I am acknowledging and agreeing that my name, log in access, webpage access, and quiz materials with results will be shared with the institution to which I have been admitted (Washington and Lee University, Rollins College, Rhodes College, or Birmingham-Southern College) pre-enrollment to populate a database analyzing ChemTutor use, and that, post-enrollment, this information will be tied to my student ID number for the sole purpose of tracking success in my institution’s introductory chemistry curriculum. Information in aggregate, de-identified form will be restricted to the Washington and Lee University, Rollins College, Rhodes College, and Birmingham-Southern College Chemistry and Computer Science Departments and, as necessary, for reporting to any grant funding agencies and/or for research purposes related to the development of ChemTutor. I understand and acknowledge that none of my personally identifiable information will be shared with individual faculty or outside my institution of enrollment and that all information used will be for legitimate educational purposes.
Participation in this research project is entirely voluntary. At any time, I can elect to have my ChemTutor usage information removed from analysis by contacting the ChemTutor team at chemtutor@wlu.edu.
